Summary:
Safe, effective transitions of care depend on more than sound clinical planning; they require an understanding of the real-life factors that shape a person’s ability to recover, engage, and follow through across settings. This session explores the essential role social workers play within interdisciplinary teams in helping identify and address those factors, while also highlighting the shared responsibility all disciplines have in supporting whole-person transitions. Placing today’s language around social drivers of health and health-related social needs in historical context, the presentation will show how social work principles have long informed person-centered assessment, advocacy, care coordination, and transition planning. Grounded in the National Academies’ framework for integrating social care into healthcare and brought to life through practical case examples, this session will highlight opportunities for stronger cross-disciplinary collaboration in support of safer, more effective, and more realistic plans of care.
