Health IT Innovations Task Force
The National Transitions of Care Coalition (NTOCC) established the Healthcare IT Innovations Task Force to bring together healthcare technology thought leaders who offer new, innovative or emerging solutions for care transitions challenges. NTOCC and Task Force members work toward the shared vision of advancing transitions of care (TOC) issues and improving patient outcomes and care quality.

The Task Force is envisioned to be a complementary and diverse group of technology companies focused on exploring the possibilities of healthcare IT – and how forward-thinking solutions can be brought to bear in addressing current and future gaps in patient transitions of care. Task Force members work on projects assigned by the NTOCC Board of Directors and the Health Information Technology (HIT) Work Group. They also have opportunities to network with other NTOCC community participants and share information on their particular commercial solutions within NTOCC venues.

Workgroups
The NTOCC Advisors Council has created four work groups to address key areas associated with Transitions of Care. Various organizations on the Advisors Council and the Partners Council collaborate within each work group to discuss solutions and create resources for use by the entire health care industry, patients and care givers.
EDUCATION & RESOURCES
On the Education & Resources work group, NTOCC is working to address the awareness and general knowledge regarding the problems associated with Transitions of Care, in order to provide critical information to various stakeholders: patients, caregivers, healthcare professionals, policy makers and media representatives.

In order to do this NTOCC is working to identify practical tools and resources that can be used by healthcare professionals, care givers and patients to improve communication in a consistent manner between care settings and reduce risk associated with care transitions.
MEASURES
Assessing and defining appropriate performance measurement frameworks which demonstrate the impact of interventions on reducing risk associated with transitional care, improving outcomes and balancing costs.
POLICY & ADVOCACY
Assessing ways to improve care through enhanced communication tools, collaborative partnership and enhanced reimbursement for transitional care support and technical medical information shared between care settings; providing advice and recommendations to healthcare policy makers and implementers.
​HEALTH INFORMATION TECHNOLOGY (HIT)
Determining gaps within healthcare information technology that create barriers to effective transitions and constructing solutions incorporating NTOCC considerations to improve transition safety.
