Healthcare transitions remain one of the most vulnerable periods in a patient's journey, particularly for individuals with complex medical conditions. Improving outcomes requires more than effective handoffs. It demands meaningful collaboration among healthcare professionals.


This presentation explores how pharmacists and case managers, working together, can improve care coordination, optimize medication management, reduce avoidable readmissions, and enhance patient outcomes across the continuum of care. Through the unique perspectives of transplant and neurology case management, participants will examine practical strategies for addressing medication adherence, patient education, social determinants of health, transitions of care, and interdisciplinary communication. Case studies will illustrate how combining the expertise of pharmacists and case managers creates a more seamless, patient-centered approach to care that improves quality, safety, and value.
