CALL TO ACTION: NTOCC Pharmacy Advisory Committee

NTOCC and the Transitions of Care Pharmacy Advisory Committee are looking for clinicians who would like to participate on the NTOCC Pharmacy Advisory Committee.

The committee mission statement: Our mission is to elevate the role of pharmacists in transitions of care by providing education and resources to strengthen their impact, collaboration with interdisciplinary care teams, patients, and caregivers, and to improve continuity and outcomes across the healthcare continuum.

The Committee is looking for:

• Pharmacists – Community, Retail, Acute & Post-Acute Care

• Pharmacy Techs – supporting care coordination & TOC

• Physicians – working in Transitions of Care models

• Nurses, Social Workers, Case Managers – aligned with TOC

We are seeking clinicians who are currently working in these integrated roles or are interested in developing the process.
