Summary: 

The 2025 updates to Medicare Part D bring significant changes aimed at improving affordability and coverage for beneficiaries. Key updates include enhanced prescription drug benefits, new cost-sharing limits, and modified eligibility criteria, expanding access to more individuals. These changes will lower out-of-pocket costs, improve coverage during the coverage gap, and introduce a cap on total spending. Beneficiaries will also benefit from increased subsidies for low-income individuals, further easing financial burdens. All healthcare providers play an essential role in guiding patients through these changes, helping them navigate new savings opportunities and make informed choices about their prescription coverage. 

Objectives:

• Review the significant updates to Medicare Part D for 2025, including benefit enhancements, new-cost sharing limits, and eligibility modifications

• Discuss how the changes will effect out-of-pocket costs, coverage phases and access to prescription medications

• Highlight new saving opportunities for beneficiaries, such as the out-of-pocket spending cap and enhanced low-income subsidies

• Identify and discuss the collaborative roles of healthcare providers in the continuum of care when navigating Medicare Part D coverage and other medication coverage changes

Speaker Bio:

Dr. Sara M. Panella, PharmD, BCPS, CPh is a Pharmacy Manager of Ambulatory Care, Population Health and Transitions of Care (TOC) for Baptist Health South Florida Clinical Pharmacy Enterprise. Dr. Panella obtained her Doctor of Pharmacy from the Ernest Mario School of Pharmacy (EMSOP), Rutgers, The State University of New Jersey. After graduation, she completed a Pharmacy Practice Residency at Yale-New Haven Hospital followed by a PGY-2 Specialty Pharmacy Residency in Drug Information at Robert Wood Johnson University Hospital/EMSOP. She started her career in TOC at University of Maryland Medical Center in Baltimore, MD and has since developed TOC programs there as well as at Memorial Regional Hospital in Hollywood, FL.  Dr. Panella has also held academic positions at EMSOP as a Clinical Assistant Professor and at Nova Southeastern University as an Assistant Professor and was previously a Residency Program Director for a PGY-2 in Transitions of Care. Dr. Panella is professionally involved with the American Society of Health System Pharmacist (ASHP) as the Chair of the Executive Committee for the Section of Ambulatory Care Practitioners. In addition, she is as a sitting member on the Board of Directors of the National Transitions of Care Coalition (NTOCC).  Dr. Panella can be credited for initiating many committees, including the NTOCC Pharmacy committee and the Florida Society of Health System Pharmacists (FSHP) Ambulatory Care Component.
