Mastery of the Social Determinants of Health: Case Management’s Next Frontier

Summary: 

The Social Determinants of Health (SDoH) are a major priority for the healthcare industry. With current costs in the trillions, these non-clinical issues encompass physical and behavioral well-being for individuals and populations, impacting as much as 80% of healthcare outcomes. The latest COVID-19 pandemic has only further amplified health disparities, posing further challenges of access to and maneuvering across the transitions of care. Complex and multidimensional client situations yield major challenges for every provider, practice setting, and community.  Through an interactive webinar experience, gain the necessary knowledge to maneuver the social, political, and economic contexts of the SDoH. The session provides a broad swipe of this timely topic, from current evidence to original tools for engaging, assessing and intervening with impacted populations. Innovative resources and programs will be discussed, along with ethical implications for case management.
