The Guiding an Improved Dementia Experience (GUIDE) Model is a voluntary, nationwide model, testing the impact of providing comprehensive services and support for people with dementia and their caregivers. Learn about the program, the deliverables and what we have learned after a year of program implementation and services.

1. Summarize the key components of the CMS GUIDE Model, including its required care-delivery services, interdisciplinary structure, longitudinal approach, and caregiver supports.

2. Examine how GUIDE creates opportunities to improve transitions of care for people living with dementia and their caregivers by addressing fragmented communication, medication-related risks, caregiver strain, and gaps in follow-up

3. Apply lessons from Gameplan Medical’s first year of GUIDE implementation to strengthen partnership readiness, care navigator enablement, shared workflows, and closed-loop communication across care settings.
