PCORI Transitional Care Funding Opportunity Coming Feb. 5

On February 5, 2014, the Improving Healthcare Systems (IHS) program at the Patient-Centered Outcomes Research Institute (PCORI) plans to release a targeted funding announcement soliciting applications to research the comparative effectiveness of transitional care services. Specifically, PCORI is seeking applications for research to determine which transitional care service clusters work best for patient populations at risk in various healthcare delivery and payment contexts by improving patient-centered outcomes and optimizing utilization rates.

The proposed research should consider obtaining the needed information by evaluating the results of the widespread experimentation now underway in hundreds of U.S. communities. PCORI is particularly interested in proposals that also evaluate the acceptability of various transitional service clusters to patients, caregivers and providers, as well as other determinants of scalability.

PCORI intends to fund one three-year comprehensive study by an organization or a consortium of organizations that has the expertise, resources and organizational governance needed to answer rigorously all the questions of interest. $15 million has been allocated for the total costs of this initiative.

The Online Application System will open on February 5, 2014 and competitive Letters of Intent will be due on Friday, March 7, 2014.
