NTOCC Launches Transitions of Care Evaluation Tool for Health Care Quality Improvement

Online software offers data tracking capabilities for care quality initiatives. The National Transitions of Care Coalition (NTOCC), an organization dedicated to improving patient transitions through the healthcare system, has launched a new tool to facilitate the advancement of care quality. The Transitions of Care Evaluation Software, a web-based program provided at no cost to the user, is designed to meet the needs of organizations ready for an advanced stage of transitions of care improvement efforts.

Available through a secure Internet website accessible from NTOCC’s homepage, the software is a convenient, online option for institutions looking to document and evaluate transitions initiatives. User features include data entry, data analysis and report generation, allowing for data visualization over time. In addition, multiple evaluation projects may be managed under a single organization’s umbrella.

“NTOCC realized early on that tools and resources were going to be necessary to support providers and systems with addressing the shortcomings in transitions. This new web-based evaluation platform represents a unique method for implementing and tracking quality improvement efforts. Using the popular NTOCC evaluation plans as a model, users will find this application an even more simplified approach to improve on transitions of care,” said Ed Davidson, creator of the program and NTOCC Advisors Council member. The resource is based upon the original transitions of care evaluation documents introduced by the Coalition in 2009.

This tool represents the latest in a growing collection of NTOCC resources, which are developed and provided at no charge for use by transitions of care stakeholders, including patients, caregivers and health care professionals.

The National Transitions of Care Coalition is a 501(c)(4) dedicated to addressing a serious U.S. health care issue: filling the gaps that occur when patients leave one care setting and move to another care setting. These transitions include patients moving from primary care to specialty physicians; moving or transferring patients from the emergency department to intensive care or surgery; or when patients are discharged from the hospital to home, assisted living arrangements, or skilled nursing facilities. The U.S. health care system often fails to meet the needs of patients during these transitions because care is rushed and responsibility is fragmented, with little communication across care settings and multiple providers.
