CMS Selects the Second Round of Sites for the Community Based Care Transitions Program

Today, the Centers for Medicare and Medicaid Services (CMS) announced the second set of sites selected for the Community-Based Care Transition Program (CCTP). Authorized by the Affordable Care Act, the CCTP provides funding to test models for improving care transitions for high risk Medicare patients by using services to manage patients’ transitions effectively.

Last year, CMS announced funding opportunities for acute-care hospitals with high readmission rates that partner with community based organizations that provide care transition services to improve a patient’s transition from a hospital to another setting. 

Seven sites were selected in November, 2011 to receive the first tranche of grants.  Today, CMS announced the 23 additional sites in 16 states across the country, which will receive the second portion of funding.  Please see the attached fact sheet for a summary of each of the 23 sites selected.
