Today, the Centers for Medicare & Medicaid Services (CMS) announced seven cooperative agreement awards to implement the Initiative to Reduce Avoidable Hospitalizations among Nursing Facility Residents. The Initiative will test models to reduce avoidable and costly hospitalizations among nursing facility residents by funding organizations that provide enhanced on-site services and supports to nursing facility residents.
This initiative, spearheaded by the Centers for Medicare and Medicaid Innovation (CMMI), supports the Administration's Partnership for Patients goal of reducing hospital readmission rates by 20% by the end of 2013.
Of particular note to NTOCC is that selected organizations will have on-site staff to partner with the existing nursing facility to provide preventive services and management of medical conditions to ultimately work toward more seamless beneficiary transitions of care, and leverage use of emerging technologies, among many other activities.
