NTOCC Launches Revamped Website

Little Rock, May 2, 2008 – The National Transitions of Care Coalition (NTOCC) announced today the launch of an enhanced website at www.NTOCC.org. The redesigned site contains a variety of new downloadable tools and information to help address transition of care challenges and to raise awareness about transitions of care. Specific content is available for various groups including healthcare professionals, consumers (patients and their caregivers), policy makers and the media. NTOCC was founded in 2006 to address the challenges and complications created when a patient transitions from one health care setting to another. NTOCC is chaired and coordinated by the Case Management Society of America (CMSA) in partnership with sanofi-aventis U.S. LLC.

Health Care Coalition Approves Tools to Improve Patient Safety During Transitions of Care

Little Rock, Ark. (April 24, 2008) — The National Transitions of Care Coalition (NTOCC) Advisory Task Force has authorized the release of recently developed tools for helping healthcare professionals and organizations address problems inherent in transitioning patients from one level of care to another. The tools represent the culmination of several months of collaboration between 29 industry stakeholders who joined together in 2006 to address challenges associated with transitions of care. In addition, NTOCC is also releasing accompanying information on how to implement and measure the tools, as well as material designed to raise industry, policy maker, media, and the general public's awareness of transitions of care. The material will be made available on NTOCC's website (www.ntocc.org/) at the end of April. NTOCC is chaired and coordinated by the Case Management Society of America (CMSA) in partnership with sanofi-aventis U.S. LLC.

National Coalition Identifies Tools They Will Pilot to Determine Effectiveness in Improving Transitions of Care

Little Rock, AR — The National Transitions of Care Coalition (NTOCC) Advisory Board Task Force met recently at Uncasville, Conn. At an October 5th meeting, the NTOCC Advisory Board reviewed an initial set of tools and resources created by Coalition working groups that will help healthcare professionals, organizations, and patients address problems inherent in transitioning patients from one level of care to another. NTOCC plans to pilot these tools over the next few months with an eventual goal of making these tools available to all healthcare industry participants by early 2008. NTOCC is being led by the Case Management Society of America (CMSA) with support from sanofi-aventis US LLC.

Little Rock, AR — Tim Rothwell, Chairman, Group Pharmaceutical Activities, sanofi-aventis, U.S., delivered an address at the Collaborative Practice Summit (CPS) hosted by the Case Management Society of America (CMSA) and held October 3–5 in Uncasville, Conn. CPS brought together a focused group of more than 125 healthcare professionals to discuss leading-edge collaboration models that explored benchmarks for measurement. Rothwell shared personal experiences regarding transitions-of-care issues with CPS attendees and discussed sanofi-aventis' commitment to be part of the solution for improving collaboration among all healthcare stakeholders.

LITTLE ROCK, AR - The National Transitions of Care Coalition (NTOCC), founded just six months ago to solve transitional care issues, continues to build support within the health care industry and continues to draw great interest from consumer groups. Originally started by 13 national leading health care industry organizations, NTOCC has expanded to include 23 groups representing participants from throughout the health care spectrum. The group has been active over the last several months assessing the various options to improve transitional care and remains committed to working together rather than in industry and practice silos to make significant changes and recommendations.
