Supreme Court Begins Review of the Affordable Care Act

Today, the Supreme Court began its historic review of the constitutionality of the Affordable Care Act. As you are likely aware, several of the Affordable Care Act’s provisions have come under intense scrutiny, and after several conflicting appellate court rulings, the Supreme Court took up oral arguments on the constitutionality of the Affordable Care Act this morning, lasting through March 29th. A ruling is expected at the end of the term in late June.

HHS Issues Final Rule on Medicaid Expansion Effective 2014

Last Friday, February 16, 2012, the Department of Health and Human Services (HHS) released its final rule for the expansion of the Medicaid program under the Affordable Care Act. Under the health law, individuals between ages 19 and 64 with incomes below 133 percent of the federal poverty level will be eligible for Medicaid coverage starting in 2014.

CMS Selects the Second Round of Sites for the Community Based Care Transitions Program

Today, the Centers for Medicare and Medicaid Services (CMS) announced the second set of sites selected for the Community-Based Care Transition Program (CCTP). Authorized by the Affordable Care Act, the CCTP provides funding to test models for improving care transitions for high risk Medicare patients by using services to manage patients’ transitions effectively.

Coalition Dedicated to Improving Patient Transitions and Care Attains New Organizational Status

National Transitions of Care Coalition (NTOCC) Becomes 501(c)(4) Organization

Little Rock, AR- February 15, 2012- The National Transitions of Care Coalition (NTOCC), an organization dedicated to improving patient transitions through the healthcare system, has announced its formal incorporation as 501(c)(4) organization. The Coalition was founded in 2006 as a partnership between the Case Management Society of America (CMSA) and Sanofi U.S. Last year, the two founders, along with the 32 members of NTOCC’s Advisory Task Force, elected to establish NTOCC as an independent, membership-based organization.

National Transitions of Care Coalition (NTOCC) Launches Three New Resources to Assist Practitioners in Addressing Issues Related to Transitions

Little Rock, AR (PR Web) February 28, 2011— The National Transitions of Care Coalition (NTOCC) is pleased to announce the release of three exciting resources: 1) the Transition of Care Compendium including the Care Transition Bundle – Seven Essential Intervention Categories and Crosswalk, 2) a health information technology white paper titled “Improving Transitions of Care with Health Information Technology”, and 3) a Brief addendum to the “Vision of the National Transitions of Care Coalition. The Transition of Care Compendium, contains a collection of white papers, journal articles, and websites that consists of resources that transitions of care-related professionals or interested consumers might find useful in a practice or medical situation. Tools and resources developed by NTOCC are highlighted for each area. The Compendium may be accessed at http://www.NTOCC.org/Compendium.

National Transitions of Care Coalition Recognizes Promising Transitions of Care Initiatives

Orlando, FL — The 20th anniversary conference of the Case Management Society of America (CMSA), on June 10, 2010, was the setting for a reception recognizing four organizations who have developed promising initiatives in their communities to improve transitions of care. National Transitions of Care Coalition (NTOCC) worked with the four organizations by providing NTOCC tools and resources as well as education as the different programs were conceptualized and developed.

National Transitions of Care Coalition Discusses the Importance of Improving Transitions of Care on Capitol Hill

Washington, DC — The National Transitions of Care Coalition (NTOCC) held their second Advisory Task Force Meeting of 2009 in Washington DC on October 20th and 21st. In addition to the Advisory Task Force Business Meeting, appointments had been made for Advisory Task Force members to meet with Senators and Representatives or staffers from their local congressional districts who are at the forefront of healthcare reform in the nation’s Capitol. The purpose of the meetings were to affirm areas of proposed language in House Bills and Senate Bills which line up with the considerations which NTOCC believes are important for better transitions of care.

National Transitions of Care Coalition Releases Health Care Tools in Spanish and French

Little Rock, AR — Today, the National Transitions of Care Coalition (NTOCC) released Spanish and French translations of patient/caregiver tools, Taking Care of MY Health Care and My Medicine List. The tools, originally developed in English, provide a resource which patients and their caregivers can use to improve outcomes and satisfaction with the health care they receive. These tools have been available at www.ntocc.org since October 2008. The new translated versions are a proactive step toward meeting the needs of the growing diversity in the U.S. population and also sharing tools with subscribers and associate members who participate globally in the Coalition through www.ntocc.org.

National Transitions of Care Coalition Creates a Work Group on Health Information Technology (HIT)

Chicago, IL — At their July Advisory Task Force Meeting in Chicago, the National Transitions of Care Coalition (NTOCC), announced the formation of the Health Information Technology Work Group. Oregon Coalition of Health Care Purchasers Hosts Care Transitions Seminar

Portland, OR — The Oregon Coalition of Health Care Purchasers (OCHCP) hosted two seminars on September 10, 2008 to bring together purchasers, healthcare professionals, health plan representatives, and policy makers to learn more about the issue of Transitions of Care. The breakfast meeting took place at PacificSource Health Plans in Springfield, OR and the lunch meeting was held at the Holiday Inn Wilsonville. Cheri Lattimer, RN, BSN, CMSA Executive Director, Annette Schmidt, Director Transitions of Care, sanofi-aventis, and Diane Waldo, Director of Quality & Clinical Services, Oregon Association of Hospitals and Health Systems spoke on four different topics.
