NTOCC is a group of concerned organizations and individuals who have joined together to address problems associated with transitions of care: the movement of patients from one practice setting to another. During these transitions, poor communication and coordination between professionals, patients and care givers can lead to serious and even life threatening situations. These poor transitions can endanger patient’s lives, waste resources and frustrate health care consumers.
NTOCC was founded in 2006 by the Case Management Society of America (CMSA) and sanofi-aventis, U. S. to define solutions addressing those gaps that impact safety and quality of care for transitioning patients, particularly seniors. The original work of NTOCC was carried out by the Advisory Task Force – an invited group of leading organizations who coalesced around this issue and discussed ways to create and implement solutions benefiting patients. The Advisory Task Force is joined by Associate Members; organizations who wish to support this important work and are in a position to advance transitional care awareness and solutions. Subscribers are individuals who wish to learn more about NTOCC’s work and use the tools the group has created.
